HOW TO NOMINATE A COACH

Online

We encourage you to nominate a coach using our online submission form.  It's simple and easy.   Please follow the links below:

Mail

1. Download and complete all sections of the nomination form (see forms section below).
2. If a section does not apply to the sport and/or nominee, please indicate “n/a.”
3. Attach any supporting documents (i.e. letters of reference, news articles, photos etc.) to the nomination form.
4. For the Major Awards Category please ensure to include the Major Awards Signature Form with your application.

Email or Fax

1. Download and complete all sections of the nomination form (see below) and submit it as an attached document by email to coaching@sportmanitoba.ca You may also fax your nomination to: 204-925-5624. 
2. Don't forget the signature form for the Major Awards nomination.
3. Please scan and attach any additional information to your email.  If faxing please indicate clearly the name of the nominated coach and a list of items being faxed on an accompanying letter.

Resubmission

Nominations will be kept on file for two years. For an individual’s nomination to be considered for a second year, the nominator must send a letter that:
(i) outlines the most recent accomplishments of the nominee, and
(ii) is authorized by the nominee for resubmission by filling out the Nominee / Nominator Information and Consent page of the 2013 Nomination Form.
